The Report Project is designed to provide a simple assessment tool for use by rheumatologists during clinical visits. The project utilizes two Health Status Questionnaires, the ClinHAQ Series 1(3 pages) and Series 2 (2 pages) and MDHAQ (2 pages) The questionnaires contain the HAQ family of functional assessments and various VAS assessment scales and are suitable for use in research, or a clinical setting. In a clincal practice, questionnaires are handed out to patients during the
clinical visit and then faxed to the NDB’s fax server for processing. The
questionnaires are verified by NDB research assistants and automatically scored.
The results are immediately faxed back to the physician for use in clinical
patient care. (Click here to see a typical report).The questionnaire
information is stored in a database formatted to provide longitudinal results
and is made available to the rheumatologist for research and patient care.
When included in an ordinary rheumatology visit, the questionnaires allow the
rheumatologist to assess and calculate the same variables that are part of the
ACR criteria. ClinHAQs and MDHAQs can also document disease severity and
improvement status required by insurance companies and governmental agencies
when new biologics or DMARDs are used. They can also help in providing
documentation of higher level rheumatology visits.
