Request a WebQuest link for current participants

If you are not currently participating in the National Data Bank for Rheumatic Diseases research project and would like to join, please see the NDB Enrollment page.

The online questionnaire, WebQuest, is now available for persons who are currently enrolled and participating in the National Data Bank for Rheumatic Diseases research project. If you sign up for this questionnaire you will receive an email with instructions on how to complete questionnaires on the Web and a personalized link that allows only you to participate. You will not be sent a paper questionnaire in the mail. Completing questionnaires on the Web is usually easier than completing the paper questionnaire.

If you have already been enrolled in the National Data Bank for Rheumatic Diseases/Arthritis Research Center Foundation, Inc. research project and you are not currently receiving WebQuest links, but you would like to try WebQuest, please complete the information below. We will then email your personal link, along with instructions for WebQuest, directly to you. In order to be certain that no one else but you receives your questionnaire we have to ask for many details below. We apologize for this, but it is done to secure your privacy.
